PLEASE SUBMIT A SEPARATE CREDIT REQUEST FOR EACH STUDENT (UNLESS IN A SEMI_PRIVATE TOGETHER), AND FOR EACH DAY.

This is for those looking to receive a credit for a single lesson in the FUTURE that they know they are going to unavoidably miss.

PLEASE… do not abuse this courtesy. When clients don’t show up for their lesson it still costs us money—The $15 fee charged to you for lesson credits, holds or make-ups, does not cover our costs. We’re happy to offer this courtesy, but it can get expensive for us when it is abused. So in order to stay in business please use this sparingly!

We have resisted putting hard and fast policies on this, because we want to find a balance that serves and respects both of us. We have a lot of compassion for the drama that gets in the way of life sometimes, and therefore recognize the need for flexibility. Help us stay flexible by considering the GUIDELINES we’ve come up with below…

HOW TO ABUSE THIS COURTESY:
* Consistent monthly request of credit/s
* Holding your spot while you go on extended vacations.
* Because you accidentally double booked yourself on your lesson day
* You were late because you forgot to account for traffic
* Poor planning/scheduling

HOW TO RESPECT THIS COURTESY
* OCCASIONALLY request credits for similar circumstances described below
* Medical emergencies
* Holding your spot for ONE week while you go on vacation
* Requests for credits that are fair to both of us

THIS IS ONLY A REQUEST FORM, AND DEPENDING ON THE NUMBER OF PAST CREDITS YOU’VE RECEIVED (AMONG OTHER FACTORS) YOUR REQUEST MAY NOT BE APPROVED.
